By submitting this housing application, I authorize and consent to the Housing Program conducting a background check in connection with my application.
This may include a consumer report and/or investigative consumer report as defined by the Fair Credit Reporting Act (FCRA).

If an adverse decision is made based in whole or in part on information obtained from a consumer report, I will receive all required FCRA notices, including an Adverse Action Notice and instructions on how to obtain and dispute the report
